Aging is accompanied by a progressive decline in organ and tissue function, increasing the risk of chronic diseases and mortality. Among the emerging hallmarks of aging, clonal hematopoiesis of indeterminate potential (CHIP) has gained considerable attention as a key age-associated condition linked to cardiovascular, neurological, and other inflammatory disorders.

In this comprehensive review, we discuss the intrinsic and extrinsic changes in the hematopoietic system that drive age-related clonal expansion and the development of CHIP. We further examine the growing evidence connecting CHIP with diverse disease states, summarize the current diagnostic approaches and management strategies, and highlight the global landscape of CHIP research.

As CHIP-centered research continues to evolve, a deeper understanding of its biology and clinical implications may pave the way for novel therapeutic interventions aimed at preventing CHIP progression and mitigating the burden of age-related diseases.
